Shoulder fracture treated

Some fractures are treated by simple immobilization with a sling for 4 to 6 weeks.

Surgical treatment is required when there is displacement of fragments or associated dislocation.

For most cases, I prefer screw-plate synthesis. Good results are obtained with the anatomical titanium plate and screws locked into the plate.

The advantage of surgical treatment is rapid mobilization of the shoulder, enabling rapid recovery. Some complex fractures require different surgical treatment.

In the case of fractures with significant, multi-fragmentary displacement, where there is a high risk of necrosis of the humeral head, a total shoulder prosthesis is proposed from the outset. The so-called inverted total prosthesis gives good results, with recovery of a pain-free shoulder and correct mobility.

The first test to be performed is an X-ray. This is performed on arrival at the emergency room.

Ultrasound can confirm any tendon damage.

CT scanning is necessary for complex fractures, especially in preparation for surgery.

symptoms

Pain is the main symptom. Patients can no longer move their arm. There is swelling of the joint. Paraesthesia in the hand is sometimes present: nerve damage.

Open fractures are rare at this level.

Definition

These fractures are common in the elderly.

These fractures may accompany dislocations.

Minimally displaced fractures are treated orthopedically. Only complex, displaced or complicated fractures will be operated on. As usual, my preference is for a minimally invasive approach to osteosynthesis.
